It's a porn game, there's going to be sex. Some suspension of disbelief is required. If things were 100% realistic then none of the games on this site would exist.

I would say that a completely realistic game can be made. What it can't be is a long game. The longer the game is the more a person runs out of realistic ways to continue the sexual aspect of the story.
The characters restrict the possibilities of sexual situations as well. So choosing an appropriate character and giving it an appropriate background is absolute necessity.
