I think it used to. I remember the first time I played it, it was there but when I replayed it after it got its Definitive Edition update it was gone. Maybe they removed it?

Perfect Alternate Ending does contain pregnancy for both sisters, Mom and Aunt
I do not remember how to trigger it however

IIRC there a two Harem Endings
The normal one without pregnancy and the "Alternate" one which contains pregnancy
I don't have any save files anymore but in the Gallery, when you select the "Perfect Ending" you're prompted between the normal and alternate one
I don't remember how it works in game though, in the "old" version I remember having to toggle something in the settings but I might be wrong as its been quite a while

Was that ending added after the game was finished?
Will guess I will just have to go replay it, its been several years since I last played it.
ADD: Yes it looks like the Alt ending was added after the game was finished.
It does not say what the ending is so I will still go play it.

For me I only want the "Completed" tag next to the game that is completed.
As for a tag for new updated just check out each game figuring out the ones you like and put them on your watch list and away you go.

Options are very limited, really. The ones I've tried and somewhat comply are:
- Life Play: Very ugly, wonky, and abandoned, but you can inbreed all you want.
- Adam and Gaia: Listed in the OP. Html, text based with inconsistent pictures, and only F/D, F/GD, etc.
- Lab Rats 2: Listed in the OP. I think is the best of the lot. Has all the flavour text, decent gameplay loop, but your kids never grow so no F/D.
- The Sims 4 modded: Limitless possibilities, but you have to mod it a lot for it to look good and be fun and lacks a a lot of flavour text (I don't think there's a mod that adds incest situations) which means a lot has to come from your imagination, and it is expensive as fuck. Also, breaks more and more with each update.

Strive for Power has brother/sister then you can sex up your kids and keep having kids to help you out either to fight or work around the map for you. (Kids will grow up once you chose the option to build what is needed, other wise they get sent out of game.)
Strive Conquest has your kids with other slaves to grow up and the same as Strive for Power.
Both are listed on the OP.
Mad Island allows you to knock up your daughters and continue to breed all your GDs.
Kinda surprised it isn't on the list.

Milky Touch has M/S pregnancy at the end if you go through her love route and don't get into any harem type situation, not sure if it's exclusively because of it, but I do know that with the extended romance mod, it even includes marriage and a wedding, you end up having 2 kids after you're married IIRC. The art style is pretty hot and so are the scenes, even has a voiced-over mom with an erotic voice and some pretty good music too. Really great game, it's one of my top favorite VN's now
Don't know if it's been mentioned yet, but Lilith's Throne allows you to breed daughter and granddaughters for as many generations as you want. It's text based but if you're fine with that it's pretty good.
