Small size of the game can mostly be attributed to clever compression. As for the content, there's a decent bit of it since this game gets new updates every month.

The incest patch does download you have to hover over the incest patch and right-click on the link and select the option that says save as and then if you like me on Windows you put it inside of the Another chance folder inside of the folder that says game
