By increasing the pink bar to the right of the screen when you are in the dungeon. Once full, you can use a spell to have a sex scene with enemies who have a heart in their name.

Hours of gameplay for sure, for me it was like at least 6-7 and it was long time ago. Game has a lot to do and unlock

Maybe endings showing pregnant images? I see a Goblin chick, there is no way she doesn't want child.

Had the same problem, wouldn't drop for me even after 10+ times, instead, I tried using the "steal" magic you get from one of the Statues, which worked to get the milk.
