I think you need to increase your sex skill which may be a bit slow, and to unlock chockers I believe you need to also advance the phone blackmail route until they ask you to send a photo with a chocker.

I believe the toys are now tied to the masturbation events. You need to trigger the masturbation scene that will unlock the specific toys.

Choosing to identify as lesbian would not remove Leo/Ella from the options possible. Trans women are women.
Furthermore, later in the bisex milestones pipeline, you'll be able to choose if you want to be with trans women or not, to further customize the experience.

Yea, it feels incredibly grindy and gated for something that essentially has minimal/no impact. Theres no "sex checks" in the game really, you dont use it to boost other stats (not that you even really can any more, its gated behind having those stats).
It doesent even take up "in game time" so its not even a resource competitor, so im not sure what the point of it is (beyond the obvious, but its very limited for that).

There are actually several sex checks. When asked to dance for the cheerleaders or the spring break, you can use sex or athletics. But it certainly doesn't see *much* use.

Also, I found an interesting issue, if you don't think the thirsty thoughts on that first instagram post you dont get to see the arousal bar. I am now seeing it on my new game but not on my old one where I was already past the Spring Break trip

Now that I've played more, I finally unlocked chokers.

You need to masturbate until eventually you get an event to masturbate in the shower, then you will get the option when you masturbate to use the shower head.

I don't think masturbating while watching porn counts for the shower event, it's only the top option to masturbate when you are horny.
