None of them show as missing. I think it's just bugged and stuck at 78/80. There are no locked icons on any of them and I can scroll through all the available tiers of the wall pictures.

Yes 100% sure. She will always find out about you and Cathy and she will break up with you. If you want the full Bella path you need to reload a save and reject Cathy on the drive home. Then you can continue with Bella. For fucking Jade, as of the end of Ep11 there is nothing that will prevent any content with Bella, but that doesn't mean there won't be in future episodes.

Updates are every 12-15 months, each one is a full episode
Writing is arguably some of the best of any VN
Sex scenes are mostly okay, the animations and composition in the earlier episodes aren't great but they get much better by season 2

No, that's a full year or more, not half a year. And each episode is multiple hours long when factoring in all 5 major paths.

What The Krogan dude said, play to the end of chapter 8 and it’s major decision time. Just play it and try it once. But, if you absolutely need to know beforehand, you’ll have to pick, between Sage, Bella, Jill, Maya & Josy, or the others path (which includes Quinn and much, much more).
