With the shocked look on Maya's and the MC's faces when the door was opened at the end of the episode it can't be just a normal looking bedroom.
Whatever they saw was totally unexpected.
(I get narratively why DPC had the door be unlocked, it led to the episode dramatic finish. But it's pretty unlikely that Bella kept the door locked previously but the night she has 6 guests over for dinner she forgets to lock it.)

I think by far the best theory is the miscarriage and nursery. Maya and the MC saw a crib and nursery items.
It's possible that it wasn't a miscarriage, maybe the child was born but died as an infant. SIDS or something.
The dead child was a girl as well.
Remember when the MC joined Jill and Bella in the park when they were doing yoga early in the game?
After Jill left with Tybalt the MC was arguing with Bella because Bella was being very protective of Jill.
The MC said "she's not your daughter" and then Bella had this brief but sad look on her face, just a second or so.

Actually thinking about in relation to the library scene in Episode 3, I can see why the MC narration talks about calling for help.

Bella is completely heartbroken over losing her child—miscarriage, or SIDS, or even somehow indirectly being her fault, hence asking James for forgiveness. It's not just James she thinks she's betraying by hooking up with MC, but the kid too—and is holding onto the desperate idea that James is still alive somewhere. Because if he can return, then so can the kid (metaphorically, I just mean have another child). Broad strokes, I can see that something like this.
I actually had another idea that I didn't mention in my original comment, but you are hovering around it with the talk of double meanings and Bella being referred to as a mom, so here it is: her child was named James Roberts Jr.

I remember a while back we were theorising how the next episode will start and I said we could see a Bella flashback that leads to her opening the door (in the past) and then it will cut back to the present for the reveal.

I'm imagining the contrast of a brightly lit child's bedroom contrasting with what it looks like now.
I posted that idea in May and the past Bella preview was released in July :WeSmart:

I wouldn't call that obvious, but very much in-line with storytelling structure. I'd bet real money on that being the case.
Yeah pretty simple to connect the dots when we can assume the next episode starts with the locked room.

There are still plenty of doubters about the room being a nursery though.

Between the house fire and the finger incident I'm not ready to make any assumptions about this game's plot based on logic. It'll either be the greatest twist ever or a horrifically disappointing and stupid decision, no in between.

Totally ready to find out it's a brick wall on the other side of the door and will never be touched on again.

I wish DPC would give us the option for Zoey to give Jacob a chance if we don’t follow her route. Tremolo moved on with his life after they drifted apart he met other girls. She should do the same and meet new people too...

Nursery is logical. The logic drops off and gets shakier the farther away you get from that.

I'll delete the game immediately and refund it on Steam if it has a nursery. Such a boring move.

That would be even dumber than making the protagonist an impotent virgin.

That's not the reason IMO. The locked room is a metaphor for Bella's past/trauma. She keeps it locked away rather than confronting it.

When the locked room is open, Bella is forced to confront that trauma which is most likely linked to James and/or a dead child.

Opening the door will be a catalyst for Bella to finally move on from her tragic past by confronting her trauma head-on with those who love and care about her. She will realise that she doesn't have to carry that burden alone.
