I guess I never saw it as that at all when it comes to the diary. For me when the diary comes up I just imagine it's the MC reading it but the flashbacks aren't his thought or imagination but just a flashback in general

Yeah same. The diary segments are not MC filling in the blanks with his imagination, they're what actually happened and are for the benefit of the player. It's a common storytelling device.
