They do during the HOTS party, they warn Chad about Sage and then when Sage talks to them about Chad, they said they understand.

They almost certainly know something, looking at how they talk to eachother about what to do with Sage and also considering they're in the same room as Chad during the HOT party and how they react there.
What they exactly know however, currently is anybody's guess.

On a more serious note. I notice this now for the first time, but my guess is that her reaction is one of the first hints we got about her wish of being a mother/possibly a lost child.
Sarah to Mel says something among the lines of that it's better for Sage to continue on knowing nothing, rather than to find out the painful truth. I don't think Mel trying to get in his pants would make Sarah say something serious and deep like that, that doesn't seem to be her nature to me.

Are we theorising that Melanie and Chad are closer friends than Melanie and Sage?
Even if that were the case, if I know my friend is cheating I am 1000% not letting them get away with that and telling their partner.
Maybe Melanie wouldn't react the same way but that shows bad character IMO.
That's why I feel like there has to be a bigger reason for her to keep quiet, otherwise its just so shitty towards Sage.
(It wouldn't matter to me if my friend was secretly gay, i'm still telling their partner that they are cheating/making them tell their partner they are cheating if I know about it).

To add onto this, i've actually had to do this myself. It was uncomfortable as hell but I learned that a friend was cheating and made them admit it to their partner. It was unconfortable but would have been way worse not telling them.

But here is the thing that is a you case, Melanie and Sarah are not that close to Sage. In fact, Sarah's proposition was Sage will graduate anyways, so no use telling her. A real friend would not even do that.
Not discrediting your experience but this is the route that melanie and sarah chose.
Its a way better theory than saying welp, she and troy are siblings so that is how they know even though when we haven't gotten any scenes or dialogue that indicate that.
