Imo she doesn't act as a bitch that much, especially if you don't flirt (too much) with her in the first 2 seasons. Even in the 3rd season overall she is quite nice, except for the moments when she's drunk. We dont know shit about their past, we only know that heather was in a wheelchair, its possible that tommy did something really great/awesome or even heroic for her in the past or that they have been there for each other during tough times we dont know anything about.

And at least partially they do fit each other as we see when she talks about her ideal date with cammy and sage (which is pizza + beer + fucking) and they stayed a couple for quite some time after all, i would consider that serious in college dating timetables and even more with a guy like tommy.

The way he currently treats her however makes it look a bit like she's just a "backup" sexpartner to him in case he needs a bootycall and cannot get anyone else. There is nothing wrong with an open relationship if it works for both partners (especially in college), but the way Tommy is acting in it is really unhealthy.
We indeed know nothing about their past, but it feels to me that Tommy abused Heather's situation to make her fall for him because he saw her as an "easy prey".
