I don't think anything much will happen, in the end Tommy will remain a pain in the ass as he is now, always angry and in a bad mood.

Derek loves Maya and was a small obstacle, Tommy can not stand Josy, and ok we are in a porn game, what problems should he have with MC? at most he could be ashamed of the relationship between Maya and Josy, but I doubt it

It's been speculated on here in the past that the reason Tommy has such a stick up his butt about Josy is that he tried to make a move on her in the past and got rejected OR he was drunk then tried to make a move. Which could explain why he has such a problem being at parties etc with her.

Brother and sister often have a complicated relationship since the dawn of time.
Tommy has a bad temper and Josy doesn't have a good opinion of Tommy's mother and this certainly didn't help

but even if Tommy was secretly in love with Josy besides grumbling as usual what should he do?

You need to use the save file where you originally bought the drawings. If you use a different one, the persistent file will indicate that they are already purchased, but you won't get them delivered by Jacob. I did the same thing. Use your save where you bought them.

That doesn't matter either. A person should do the right thing, regardless of the consequences. If you are in a relationship with Sage, you owe her an obligation to look out for her interest. There's a chance that Sage will even blame the MC for telling her, but that doesn't matter either. If you are a good and moral person you should do the right thing.
