I don't know, I don't have a MacOS. That's how RenPy compiles.
P.S. Maybe you need to create the BigBrother.app folder and move the game files into it. I thought I didn't need it and removed it (not the contents of the folder, but the folder, the first level). I don't know about that.

I had the same issue. after the unzip is looks like a folder. Change the folder name to end in .app
for me after that the icon changed into the familiar safari kid and snake and the game was playable. The file now reads as BigBrotherRenPy-0.12.app

2. Go to "Настройки" and click on the English flag.
3. During the game, select Max's phone and press the switch at the bottom.

How did you install fix that? What do you mean, it's not working? An error pops up, so where's the screenshot or text of the error? Does the game not start or during the game? Was the game working before the fix? You used the Unren?
