It's triggered by money threshold. If she has less than 100 essies when going to the pharmacy it will trigger the sex scene.

Regarding the narrative paths and the corruption element, I understand that things might seem confusing at first. However, the goal is to let you guide Yana's journey in the way you prefer. She isn’t portrayed as the typical naive American/Japanese naive girl, she already has some experience due to Eastern European life being quite tough. In the end, you have the freedom to complete the game without any sex, or you can choose to leverage all her skills and experiences to achieve her goals.
