From 0. You could probably load an old save but there's no guarantee it will work without bugs/glitches.

Public Service Announcement
The latest version (0.15) requires you to start the game from 0. There's no guarantees on what will happen if you load an older version's savefile and doing so expecting something to not go wrong is gonna be a fool's errand.
Also the intro has a bug (if you choose not to skip it it loops, according to a Discord user).
The Doc will upload the fixed version tomorrow (or in around 12 hours) and go live for both Subscribers and us, as the current version (for Subscribers only) has that bug. Here's the word from him on that issue after Kraehe let him know about it and the Doc checked the code:
I'm posting it now so people know about it (if the new version leaks unofficially) and to try to avoid comments about it since it's a known bug to him after it was released.

Hate to be a critic, but "Some time later" cut screens are too common and slow down the grind more than it needs to be.
