tried deleting the bonfire folder in appdata, re-extracting the game, nothing works

That is very strange and this is pretty much the only instance where, after the full cache reset, that quest doesn't work. You can try sending me the character file for debugging but I don't think it will bear anything usefull if it really is a brand new Erol on a clean cache.

oh I used my old character, will try and use the default one one last time as soon as I can.

2.Open Bonfire and choose your character, Start game, Input instruction:

Exit

3.You will see your item bar added tattoo props, You can put on or unload.
