On the next update after 3,4 months . Probably NTR again we all will see Karen tied in the Gents Toilet in the Collage. Her Roads are already broken. There is no fix for her in the game. It is ok to have NTR content but at least it will be good to Follow some of the Main and Characters Story. no, the original game doesn't have actual incest and any built-in ways to switch it on with quick patch. Even though the situation with guy moving in with the older lady and her daughters reeks of it(good ol' landlady and roomies).
Gotta wait for some fine soul to comb through various UI, journals and the talk scripts to adjust it manually, and I'm not sure the original guy is still into it(he probably lost interest after slow and NTR-focused updates)

Those people are downloading a save file with options already set. If you play your own game and progress yourself that stuff won't appear.
No, it's 50/50.
