Fattening Career has a lot of Breast Expansion content now.
There are now four characters who are experiencing a lot of growth in pretty much only their breasts and a few more whose boobs grow huge, but also other parts of their bodies.

Just found this game, , recommended to me on itch.io. Currently signing up for the patreon, will report back with my findings, but it definitely has BE and lactation. By the way, Patreon version just has an in-game walkthrough, that's the only difference. I just like the cut of the designer's jib and wanted them to have a little walkin' around money.

While the growth can be a bit slow Aruna and the Labyrinth of Sealed Lewd does have some damn good body-modding. Even has (optional) futa for those of you interested
I enjoyed this one as well. The trick is to go through most of the training levels (There's 15 total I think). Completing them unlocks different options in the equipment selection including an ever increasing development multiplier. I was pleasantly surprised by the ass/thigh expansion option which games rarely ever have.

Game's a pretty standard Ren'py Harem VN. The BE is alright, but there's no process, just instant giant tits. Size goes up to torso, BE is limited to only 4 characters right now, and comes along with a kemonomimi-style cowgirl tf. MC has to milk the girls to reverse the transformation. Lots of sibling and cousin incest as well if you're into that.

The writing is good, and the MC is a decent person who's trying to help his family and community. The tone is kind of sweet, really, for a game about a dude banging every woman in a ten mile radius. There's also one heroine with quite a bit of pregnancy content in the most recent patch if that's your thing.

All in all, I think I'll keep my eye on this for a while. Looks good so far and I'm honestly excited to see where it goes. Might even kick up to the $50 patreon tier to get a custom OC in the game.

Yeah I understand this sentiment, in my own game I'm really unsure where the line between too much work and not engaging enough is for a lot of players. I do like that a quest allows players to make decisions that can change their playthrough and enhances replayability. Personally, if my game ever contains quests, I'll probably add a skip button so that people can get to the real content they want to see.

I think a game like Cowtastic Cafe strikes the perfect balance between how fun the game is and how much BE there is, but that's not really a VN.
There's several things a lot of devs tend to fail at when it comes to lewd content in NSFW games, yeah:
1. Where the player's eyes go. I hate it when you've got a lewd game that requires a lot of focus on the gameplay element, and the game element is away from the actual lewd bit. Example: Fun game (and thread-related), but as the puzzle part of the game speeds up, you slowly become unable to look at the girl, it gets more risky to take your eyes off the game itself as you don't want to risk losing, especially since Dr. Mario screwups are basically exponential as more junk piles up. There's definitely worse games to use as an example for this, though. Another example: Good fuckin' luck playing the game while trying to see what the women look like when they're fat, heh.
2. Gaps between lewd content. Devs like to pad out content to make their games seem "longer" for some reason. It's not like players are paying a monthly sub or anything, but devs will add in things like farming a character's affinity for several in-game months to unlock the next scene, or grinding the same job mechanic to amass enough money to buy the gadget to unlock the next scene. Example: The game's pretty bad in general, sure, but it's made worse by how you're stuck walking around randomly, chatting with everyone just to find the next scene, then THAT scene is just a giant text dump about nothing which then leads to another text dump about nothing, which leads to a text dump about how you need to talk to [x] to maybe fix your magic so you go to another text dump and you finally get a short BE scene, then more text dumps.
3. In-dev games with no debug menu and no way to skip to the new stuff. Bonus points if saves aren't compatible. Dev updates game to v0.6, saves aren't compatible and WHOOPS, now you've gotta play the first 4 hours of the game again, just to get to (and bugtest) the new content. It's frustrating for the player, and it means bugtesting is basically non-existent, as only the serious players will get to the new content at a decent rate, it might be a WEEK before bug reports start coming in. Bonus points if the game's hard or really long.
4. Easy games with the lewd content locked behind losses. Sucks when you play a game and get zero lewd content, then later find out you're supposed to lose to things to get the sex. Bonus points if there's no gallery unlocks when you win. Double-extra-bonus points if there's no way to know if something will unlock on loss with an enemy beforehand, and there's no easy way to lose on purpose, like a "submit" button, so you're stuck running into every single fight, defending again and again so they slowly chip away at your health and you hope and pray that you get a scene out of it and that the scene itself is something you like and not a turnoff like the time I got surprise-cockvore'd in a game a while back by the one enemy in the whole game that cockvores.
5. Hard games with the lewd content behind wins, BUT the game's difficulty is simply a grind check. This happens a lot with hard RPG Maker games for some reason. It's not that the player needs to play better, no, they need to go do a long side quest to unlock the special move that makes the boss possible, or they need to go grind for 8 hours to get three levels higher so they can survive a hit from the boss, or they need to grind for 4 hours to get the money to buy new equipment so they can survive a hit.
6. No advance warning for an upcoming fetish. Pretty common with sex sandbox games like CoC and TiTS. You meet a woman holding a pump. You talk to her. Is she going to use it on herself? No, she inflates YOUR chest into boobs, which sucks because you're playing as a guy! Or maybe she stands there and you think to approach her, but then UH OH, she drops her pants and begins inflating her dick! Better have 53 AGI or higher or she's going to cumflate you!
6a. Unrelated kinks. Just kinda sucks when you've got a breast expansion game, but then there's a vore character, or a fart fetish character, or diapers or something, completely unrelated to the rest of the game. Makes me think they were added via a Patreon goal being achieved.
7. Trying to force in drama and complex plots and such into a lewd game. Gotta remember, players currently have their hand groping their genitals, trying to make them think too much or cry is simply going to make them skip to "the fun stuff". Very few people care about the plots in porn anyway. Also see #2.
8. Two handed porn games. Again, players have a hand on their genitals. Pls stop making games where players need to keep their hands on the controls for the whole game. At the very least, have a way to one-hand the game during lewd scenes, or autoplay for no hands at all. Surprisingly, most RPG Maker games these days have mouse controls.

I'm drawing a blank on more observations; my caffeine levels are dropping and I'm going downhill, heh.

Pretty much spot on.
I've tried alot of games, and most of them come out as too grindy. When i play transformation games, what i want is to see those scenes. Not to spend 99% of the time grinding or spending it reading text.

Hypnosis is an example of a game done right. There isn't much time between each lewd scene. The only mistake it makes is shoving the big BE changes into latter quarter of the game.

Fattening Career is also done right (with a flaw), it is abit grindy to get money though its not bad. However the creator added fetish scenes to it so it works out. It's the same deal with the school, bit grindy but it has fetish scenes.

The Gain Trials is a game done right. It has a short story, almost all of its content is fetish scenes. No grind as they naturally "grow" as you watch the fetish scenes (which can be skipped to get faster progress).

The Repurposing Center: Another great game. There's "grind", but it contains lewd stuff. The jobs to earn money involves lewdness and body modification so you can spend it on more body modifications, clothing or other stuff. It's all connected and thats awesome. This is a game done right.

There's other games which does things right. The above serves as examples though.

Then there's games i just gave up.
Housewife Simulator is an example of this, it wants you to grind....alot. It was a waste of money.
The Growth Factor Game is another with alot of grind (but atleast its the easy kind).
Housewife i just gave up, it was a pain to play. Growth Factor is bearable, but it's far from rewarding enough for the grind it puts the player through as i feel its minimum effort on content.

In short, games that takes away from the lewdness (be it kinky, sexy, body modification scenes) is just plain bad. Especially when they seem to force the player to grind a ton.
And that grinding is something of a topic these days as even AAA studios make that mistake.

To put it this way:
Walking from Solitude to Riften in Skyrim can be fun because it has a ton of things to discover on the way.
Walking from Little China to Pacifica in Cyberpunk is great because Night City is incredibly beautiful and well crafted city (and it has stuff to be discovered)
And BOTH games features the ability to SKIP by fast travelling.
This is "grind" done right.

Sometimes creators write - Add Sofia new images. And you see where is she in your 5 girls... ou yea on 3 minute of start game you can take 1 dialogue wana take her number of phone or not.You load save.You dont take number. You grinding in hope to seewhere is she.Gross.

As an example. How long time you need waste on every girl several times... But game is good.

Possible fixes to these issues:
1. If possible, have the UI close to the thing you want the player to enjoy. If it's a BE rhythm game, have the player click her boobs, or have the arrows come in from the sides to meet up at a point on her chest. Or the safest bet: leave the UI as-is but add in pauses during the gameplay so the boobs can expand and the player has time to look. Or add a gallery that lets you look at all the sizes you've unlocked so far.
2. If two lewd scenes are far apart, add a way for the player to do an optional lewd scene in-between, or add some lewd scenes just out-and-about. Got a belly inflation game? Just plunk some woman down in the middle of town doing a cycling inflation animation, or add some woman getting cumflated by vines in the forest. Or just plop down some simple side quests for the player to find that result in a lewd scene.
3. Add a debug menu. Or a "cheat" menu. One button for infinite money, one for gaining 10 levels, one for skipping to the game's newest chapter, one to simply spawn in a quick bad-end lewd scene to look at, whatever. Add a portal in the starting town that spits you out at the newest town at a recommended level with all the story flags completed and 999999 gold.
4. Unlock the scene and all its variations in the gallery if you win. Hell, make the gallery a Patron reward if you have to, but unlock that scene without having to lose. Either that or make the game/fight harder, I guess.
5. Make the difficulty more skill-based rather than just DPS checks, or add an easy mode. Lower the boss's stats, but give them a super move with a tell that lets observant players know to block, or have an NPC somewhere talk about some sword that can beat the boss, and give a hint as to where it might be, the sword making the boss way easier.
6. If a character has a dick, add a bulge. If they cumflate people, add some prior victims that look like cum balloons. If they skip the foreplay and abuse their victims to death, add in bodies, or an NPC going "oh god, I saw [x] raping some dude's neck! Don't go in there!" If it's a text based game, make sure to signify who's doing what in the option. Like damn, playing a text RPG and picking the "anal" button; whoops, the game forgot to specify that YOU are the one getting fucked. Pls say "give anal" or "fuck their butt", not just "anal".
6a. Bro, if you've got a realistic breast expansion simulation game, just... don't include unrelated fetishes? Milk, sure. BE, sure. Belly inflation... probably not, unless maybe someone's chugging 10 gallons of breastmilk fresh from the "tap". Helium-filled boob floating? Uh, okay, but keep it reasonable and few, assuming you can fit it within the game's setting. Scat? No. Muscle growth? No. Hyper WG? Probably not. Use common sense.
7. Again, no complex plots unless you've got a fanbase built around your lore, and even then keep it to a minimum; most people don't give a shit and most will outright forget character names mere minutes after their introduction, especially in lewd games. If you're gonna do something like this, at least have it tie into the lewd, like (a belly game) or (just a sex game.)
8. Click-to-move. UI elements we can click. Platformers where UP is jump and DOWN is crouch (or W and S). Sex scenes we can set to play automatically, or ones where we advance it by pressing any key. Hell, give us a babymode with autoattacks, so we just walk into enemies to fight them or something, heh.

Abso-fuckin'-lutely. To (very roughly) simplify, a lot of these games just forget the audience they're catering to in one way or another. A5 steak doesn't interest a vegan, no two ways 'bout it. It's why I genuinely believe Cowtastic Cafe is one of the best BE games: the BE is just as important, if not even more so, than the rest of the game you're engaging with. Controls are simple, the game feeds into the appeal, which feeds back into the game. It all works

Now rq, I DO want to minorly defend Dr. Bobs with one simple statement: the game pauses when you hit a threshold. That's all, the rest is pretty accurate, thank you for saying what I always thought but could never properly address

Dev updates game to v0.6, saves aren't compatible and WHOOPS, now you've gotta play the first 4 hours of the game again, just to get to (and bugtest) the new content. It's frustrating for the player, and it means bugtesting is basically non-existent, as only the serious players will get to the new content at a decent rate, it might be a WEEK before bug reports start coming in. Bonus points if the game's hard or really long.
4. Easy games with the lewd content locked behind losses. Sucks when you play a game and get zero lewd content, then later find out you're supposed to lose to things to get the sex. Bonus points if there's no gallery unlocks when you win. Double-extra-bonus points if there's no way to know if something will unlock on loss with an enemy beforehand, and there's no easy way to lose on purpose, like a "submit" button, so you're stuck running into every single fight, defending again and again so they slowly chip away at your health and you hope and pray that you get a scene out of it and that the scene itself is something you like and not a turnoff like the time I got surprise-cockvore'd in a game a while back by the one enemy in the whole game that cockvores.
5. Hard games with the lewd content behind wins, BUT the game's difficulty is simply a grind check. This happens a lot with hard RPG Maker games for some reason. It's not that the player needs to play better, no, they need to go do a long side quest to unlock the special move that makes the boss possible, or they need to go grind for 8 hours to get three levels higher so they can survive a hit from the boss, or they need to grind for 4 hours to get the money to buy new equipment so they can survive a hit.
6. No advance warning for an upcoming fetish. Pretty common with sex sandbox games like CoC and TiTS. You meet a woman holding a pump. You talk to her. Is she going to use it on herself? No, she inflates YOUR chest into boobs, which sucks because you're playing as a guy! Or maybe she stands there and you think to approach her, but then UH OH, she drops her pants and begins inflating her dick! Better have 53 AGI or higher or she's going to cumflate you!
6a. Unrelated kinks. Just kinda sucks when you've got a breast expansion game, but then there's a vore character, or a fart fetish character, or diapers or something, completely unrelated to the rest of the game. Makes me think they were added via a Patreon goal being achieved.
7. Trying to force in drama and complex plots and such into a lewd game. Gotta remember, players currently have their hand groping their genitals, trying to make them think too much or cry is simply going to make them skip to "the fun stuff". Very few people care about the plots in porn anyway. Also see #2.
8. Two handed porn games. Again, players have a hand on their genitals. Pls stop making games where players need to keep their hands on the controls for the whole game. At the very least, have a way to one-hand the game during lewd scenes, or autoplay for no hands at all. Surprisingly, most RPG Maker games these days have mouse controls.

I'm drawing a blank on more observations; my caffeine levels are dropping and I'm going downhill, heh.
Was also going to write a short list of fixes for these issues and also add in more example links to games, but... zzzzzzz...
This was a great writeup, considering your eye for these sorts of problems could you maybe shoot up a list of your fave BE games? or huge boob games in general. Not expecting a big review of each one, unless you're up for it of course!

for me works this:
download and install it, then on the game.exe rightclick and > locale emulator > run in japanese (admin)
it load short an opens
