Probably Straitened Times. Not a bad plot (but not perfect, there are questions about it), plenty of content, but not a f**kfest. The only thing that can be criticized is that some events happen there based on the choice of random number generator. Although for me personally (Nota Bene!) it was not a problem when playing this game. Yes, it is important to install the incest patch (even though I'm not a fan of incest) - otherwise the plot will turn into the ravings of a madman.
Probably not unless you use something to go into the code and reset the to default; which is basically what the cheat did, change the setting on that.
So the arousal is show by the hearts on the screen. You need to max those out and then not do anything sexual that will drop them. Sex resets all those back to zero. The definitive way is to find playing cards; each of those raise your arousal. Now there are also other game encounters that raise it as well. It's been too long since I played it to tell you which ones there for that. But one other problem might be what else you need to work on as well. The way scenes were added to the game means some events don't trigger unless other challenges have not been completed to a certain level as well. If you have gotten to max arousal and still can't trigger it, I suggest doing other events and also messing around with the time you go to bed. I have found timing can be an issue as well; for instance, if you go to sleep too late (past what time the event trigger is looking for) it won't trigger the event as well.

There is such content in the game, but I can't say it plays a leading role there.
