yes, you can have dick, have no dick, a vag, or both. the game also has a femininity scale with 0% being guy and 100% being woman which denotes most of the usual things like hip and waist proportions, breast, ect

you can also change the term for each gender if you dont just wanna go with standard "shemale" and whatever the inverse was (i always forget)

Well, do the drug transformation mission (proceed with Eliza's quest), where you will force collect the drugs from anyone in the prison, then after capturing the drugs, bring to Eliza and will be given a choice to try the drug on yourself or someone, before they scan you for the affect after transformation
