Oh, you're right, the other Adrian in update 19 can fuck MC with a magic peg/dick. It's in the endgame gallery once you've unlocked it in game. For this Adrian needs to be in your party.

Whats the endgame gallery? Do you have a save with it if it wouldnt be a bother?

It's just a bit frustrating, you know? Apparently you don't play the game. So you don't know the twist of update 19. So you don't know know what endgame means. Have you played the game before? Have you seen all the scenes where MC can be bottom? Have you seen Adrian fuck MC after one of the possible cart race outcomes? Do you know that the Other Adrian doesn't have a dick?

Here, take this save and be happy. The endgame gallery is in the menu after loading the game. Have fun. Whatever.

There is no incest patch. You can choose how you call Lisa and Suzy in the first minutes of the game. MC's biological mother died when he was a child and it's an important part of the story.

Quote from my own review about incest in Come Home:

- Incest fans like me have to turn up our suspension of disbelief a notch, because the story is written in a way, that MC's actual mother died years ago and he comes home to his not-really-step-mom-because-her-and-daddy-never-married and her daughter from an earlier relationship. You CAN change the way you call each other to sis/bro/mom, and 95% of the time that works fine, but sometimes the story talks about your (deceased) mom or something similar.

It's also in the FAQ that MC and Lisa/Suzy are neither step- or blood-related.
