The cup size is the displacement of the breast material itself. So a say 24D is going to look a lot more massive than a 34D but both have the same amount of breast material. This is of course if they have a properly sized bra, and I'm tempted to say most women actually don't get properly fitted and just wear whatever feels right off the rack. It becomes a lot more important when you have larger breasts though and don't get me started on underwire.

The most accurate is going by cc's of fluid displaced. For a not at all random example a G cup is about 1200-1300 cc's. Your biggest problem with CoC2 though is they are extremely inconsistent with size descriptions and the bust pictures.
If someones biggest problem with CoC2 is the inconsistency of picture's descriptions, they should be considered legally blind
