It is definitely not a "Chad Thundercock gathers a harem of virgins" type of game.
And was initially designed with female protagonist in mind. So preventing your companions from fucking everything that moves can be annoying when playing as a male protagonist. But you generally have to pay close attention to what characters say, and if it looks like they're getting too chummy with someone/something - you pick the refusal option(s).

Yeah I can tell even from the early game that its Fem-coded asf, but there are a few options.

It's an old game (by the industry standards), made by old people. From the time when western porn games in general didn't have a set artstyle, but instead just grabbed whatever art commissions they could afford. RS:A doesn't even have a main artist, the art was done by about a dozen different artists over the years.
I guess that AI-ifying was an attempt to "bring cohesion" of the "artstyle". But it's easy to fix by replacing the pictures folder with the old art, there's even a link in the main post to a guide on how to do that.

One other relic of that wild time is "Adam and Gaia". It's on f95, and you can clearly see in what kind of environment westen porn game industry was back then.

The companions do sound like they get real carried away tho.

Don't know if that's why I feel like CoC2 is a bit of a laugh or not but since it doesn't do a particularly good job at what I would care from it and it seems pretty focused on doing things I couldn't care for, guess that's really all that's left.

Fate's unavoidable heroine rape is treated like rape and the scenes put a lot more emphasis on the psychological torment and damage that does to all involved. Sakura is pretty emotionally defeated by the time the game even starts.

You, the player, are very clearly meant to hate Shinji (really that entire family minus Sakura. Probably.).

I imagine it's Shinji on Sakura you're referring to (I think there's a Medea on Artoria bit too but I haven't really read much of FSN, I'm still reading Tsukihime), in which it's very much treated the way it should be. Hell, it was so strong that Shinji hasn't had more than one depiction in Fate-related medium where he isn't crap - that being Extra CCC. Shit, even the "Shirou's cooking show" anime only shows him once as a secluded, distant character because they can't do jack shit with him because of him being such a shit.

He's in such a state that even his grandfather showed up on two story arcs of the gacha, meanwhile he's relegated to being the face of some shitty CEs that love to show up at inopportune times.

The only true unrotten egg of the family is Kariya but he suffers from a lack of competence and then gets saddled with a Berserker, who's bound to fuck him up in one way or another. Sakura doesn't even count because she's from Rin's family, her father gave her away so she could be "raised properly" and he had no idea on how fucked Sakura's raising would've been.

The mere idea of sitting down and watching a character go through the motions of being repeatedly betrayed goes exactly against what most folks do and it's why that's divisive.

Take Rent-a-Girlfriend for example, a manga that can mirror CoC2's talent for swerving from interesting perspectives that I would like to see, where the main protagonist repeatedly hires a girl to act as his girlfriend, repeatedly finds opportunities where he can improve, even finds a girl that genuinely has interest on him that would crash that web of lies he threw himself into.

And then he collapses. Sinks like a stone, just like he knew he would. Just like the reader knows he would. Repeatedly. Into nothingness and memery. You can give a character the benefit of the doubt if he fucks up one, two, three times - make it a habit and you basically know all there is to it. Fuck Rent-a-Girlfriend.

TLDR, most people don't like being betrayed. Being betrayed repeatedly makes them feel like they're wasting their time and tying the tag NTR to anything is a good way to push those people off. Do it during production, shit flies.
