Ya i had everthing ready to go just couldnt remeber what the action button was to get the fish.

You should find in the hallway preceding this room a sign with 4 symbols, which are also inscribed on each of the spinning wheels in the picture. Turn the wheels to reproduce the sequence and a new path will open (you can check the order of the spinning wheels by the number of dots they have at the bottom, 1 dot is first in the order, 2 is second, etc.)

Later you will find another sequence of the same symbols and must come back to change it to that and another path will open

Yes this is a bug that will be fixed in the next build.

I love that idea, had I the game development skill I've been meaning to do something similar for awhile. I even wrote an ultra short story that toyed with the idea. So I'm happy to see that someone else has thought of it as well.

Honestly storywise i tought it was like a timeloop thing where stuff happened but the body gets "respawned" back by the mcguffin necklace taken at the start of the game while chippin away at the MC sanity with each death that he remembers (kinda like Connor in that SCP animated show made by LordBung)
