To clarify my understanding: The Uncle Peter comic begins abruptly with him mid-conversation with a nurse. In the second panel, there's a sort of "flashback" to a scene that doesn't appear in any other comic, correct?
Are we to assume that Uncle Peter went for a check-up and, for some unexplained reason, the nurse performed oral sex on him? Or did this event actually occur within the context of another comic in the series?

well we don't know exactly where this comics is located in timeline, but I think it is before the moment when Peter appeared in Sarah's House, because when he first time showed up in comics he said doctors give him strict guides what medicine to take . what about nurse Peter as usual blackmailed woman. he said if they don't have sex he will not ask his friend to give nurse's husband a promotion

Now, about the story, it's really comical, Daval just has no idea how to write an incest comic in an organic way, it's full of cliche like the whole "he's hard, but I don't want to embarrass him" or "it's my fault he's hard, so I will help", it's clear Sarah is just a whore that uses any excuse to be close to a cock, also the fact that this issue end in a sexual cliffhanger is proof enough that Sarah and Aaron will fuck in the next one, I wont mind at all 150 pages of just them fucking over and over again, but I hope there's some David/Alice development too.

Why is Sarah a "whore"? Any more than Amanda, Alexa, Emma or even Lana? What you've described is literally akin to every set up Daval has executed in his other comics. As much as I'm addicted to Immoral Desires, the set up is that Lana receives a dildo from some stranger (Daniel) whom she assumes is Sofia, and convinces herself that she'll get better at giving her husband blowjobs by practicing on her son. Amanda literally lets Liam eat her out in Chapter 2 for no reason, and has sex with him just as quickly. Alexa, pretty much speaks for herself, indiscriminately has sex with her nephew for internet likes and attention. Emma had sex with her father, literally moments after being sodomized and inseminated by Liam. Sarah early on let David ejaculate between her breasts because he told her he saw someone's aunt (Alexa) receive a facial from her nephew (Liam.) Alice ate out some random woman's pussy in her workplace bathroom. Most of these female characters place an inordinate amount of value on immediate sexual gratification in these comics, so why is Sarah, particularly, "the whore"? I agree with you that these set ups are uninspired, and perhaps lazy, but the same could be said for everything else Daval does. We knew eventually that Sarah would get involved with Aaron, so this should come as no surprise.
I think the fan base still hasn't forgiven Daval for MT Chapters 4-6, and that's the reason, in my opinion, Sarah's always getting singled out despite the fact that her preoccupation with immediate sexual gratification is comparable to that of Amanda, Alexa, Emma, Alice and Lana.

Because I was just talking about Sarah, but you are right, in fact all of them are "whores" because Daval just don't know how to writes an incest story without the cliches, the development will never feel natural/organic because people always complains about lack of sex in those stories.

While I wouldn't call them "whores," I actually agree with you. A lot of these set-ups are in fact cliche and filled with tropes. Sometimes that's a good thing, because repeating what works helps draw in the audience, but in other instances it makes the characters one dimensional. I also agree with you that the audience may just be as much to blame as Daval is. I'm sure there were many in his ear saying something akin to "we want sex now." I've always thought, for example, the sex in Immoral Desires was rushed--though by Daval's standards, it was delayed since Sarah and Amanda both had sex with the MMC in their respective second chapters. Imagine if he took the time to flesh out, and slowly escalate the sexual acts between Lana and Daniel, i.e. handjob in Chapter 2, titjob in Chapter 3, blowjob in Chapter 4, assjob and intercrural in Chapter 5, and finally sex in Chapter 6. Imagine the room he would have had to work with had he done it this way. Sex in these comics is like sex in real life: once you rush it, all that's left is to find ways to keep the excitement alive, or become complacent with the repetition.
Dustellar, how would you have set up Sarah and Aaron? I'm genuinely curious. I had some ideas, myself.

That page you talked about is just a flashback of the previous day that doesn't have anything to do with other comic but would be cool to see how Pete convinced her to give him that bj in a prolonged flashback, I saw a lot of fans confused about that too.

I honestly think this is wrong on Aaron's part, and it's the kind of attitude that made many people hate the old Peter. Was Daval trying to make the boy a new version of him ? Honestly, I don't think it's necessary. Aaron is a character who can grow on his own without following in Peter's footsteps, like Daniel in ID. And this just shows Aaron's hypocrisy even more, abusing Alice while she slept and angry because his mother fucked David lol, there is no saint in this story
