Definitely the hair cap. You just need to adjust it's size. There's usually something to do that in the hair's settings. I believe making it larger so the cap (which normally isn't visible) doesn't clip the characters head.

No need for meshgrabber. Just adjust the hair cap's size, usually make it larger. This happens when the cap clips the head.
