The gameplay will have to address the heroine walking around with a bloated belly and not being able to fight or go to school or play card games or whatever, and that will almost certainly disrupt the core gameplay loop. And then once that's done, what does the game do with the baby which is now presumably an important consideration in the heroine's daily routine? Generally speaking; pregnancy is a life changing event that causes emotional, physical, and ethical upheaval and is added to scenes/dialogue to add gravitas. Usually it's the endgame in terms of RPG mechanics; hence even if the game includes such niche content it'll usually be an ending.
I think I've only seen it down "well" in two games, Hazumi and Pregnation and Goblin Conqueror (which is more of an extended scene). 99% of the time it's there as a joke to shoe in pregnant belly sex content (which is totally fine). Also, occasionally it's some monster egg laying thing that doesn't follow normal gestation.

Although animations and CGs are a bit disjointed in terms of dialogue/content the gameplay was passable. Unfortunately there is only impregnation without follow through (no scenes or follow up) and the game would've been way better served with BF mechanics liken Colony City 27.
