If you ask to the townies really few of them born in the "natural" way..
Most coming from the birthing program and there's also more about that when you will recover the artifact for Sagara's quest, if you give it to them (can be consigned otherwise to Emily or Caith, that could lead to end the job as spy)..
Infertility should be enough common in that dystopia..
Missing protections, condoms, mestrual cycle (tampons and relatives)..
Not saying is impossible, but will need a lot of work in case he will change his mind about it..

I have chosen 5 fetish at the start of the game so i get 5 more points to use for stats allocation. After a while, due to unfilfilled fetish, i acquired some anxiety.
Thus i did some testing about them trying to gain understanding of the effects of fulfilling the fetish.
I have chosen fetish that are easily to achieve, like anal, edging etc and found out that even though i can give NPC pas anal (rec), i cannot gain any act anal fetish points even after i submit to pleasure and orgasm.
I tried anal with both female and male NPC and still i cannot gain act anal points, and wonder if this is a bug?

Uhm.. there're fetish for both receiving anal and giving..
Perhaps you choose the wrong one?
With a save would be faster to say..

This is how fetishes should work and where to find contents for them..
When you orgasm all the previous acts should be considered..
In almost all my runs i'm "virgin" for really long time cause i tend more to exploit little bugs..
Will try to explore more the sexual part.. but i find it really boring because of the limited choices lol..

For the giving part there is no message about "Moderate fetish triggered (pas anal training) "
I got the "Anal training (rec.) +2.0" from receiving, but have nothing when giving active anal.

Yes, could be something bad happening when you choose both the options for those..
I recall i noticed that while those were giving still a lot more of anxiety & Lust, reported it then never really checked if Rax solved it cause he was going to rework the fetish system..
Try to choose just one then see if you can train the other, for now, until he will dig deeper in those..
Rax hates when i report bugs experienced by someone else.. expecially when coming from this thread!
Also for that i keep saying "report it on Discord"..
I guess i will do a quick run just to exploit it and report, if you hate Discord..

Need to raise their Submission bar in a way or another.. Gaining their trust usually helps a lot..

Would need a save to understand if something wrong..I got an NPC's submission to 100, and I still can only do basic dom stuff with her
I can do things like slap her, spit on her, and the like. But I can't figure out how to move on to stuff like prostituting her, putting her in a chastity belt, getting her modified, etc. Hell, she still tries to seize control during sex.

Need to understand if something in NPC starting personality is interfering too much..
You need to access her inventory to make her wear a chastity belt.. you need to have it in your inventory..
With hormones you can modify Ass/Skin/Breast/Vagina/Cock/Height..
Those can be bought from the researchers..
The first step is meet Rebecca/Reban, gain their trust then get back the sample of the Equiknot DNA for them..
(You can meet them in Pangolin or Underground Club -> abandoned house after midnight.. accepting to be a test subject for them will lower anyway your status in Isara)
After giving them the sample they will talk about their Boss Sagara..
Once accepted the job as Spy in Isara you can start helping the researchers..
There're 3 of them.. Anna, Zoe (Isara) and Ciara (Pangolin)
Each of them will offer you 2 quests, every quest you will complete will unlock 2 hormones related to that quest..
