What do you mean, a second time? She takes him to the hospital when the player selects that it is time to see the doctor at the beginning of v0.04.1. The save in slot 50 starts at v0.04.1.

So, I'm aware the game dev is quite active on this thread, and I'm not sure I'm overstepping my bounds by making "scenario suggestions" here but I'm pretty sure I've seen other people make suggestions in this thread so I'm just gonna shoot my shot. Feel free to ignore if this idea doesn't interest you.
I feel orgasm denial could be implemented in (one of) the bad endings. It was touched upon briefly when Lilith made it so the MC can't orgasm without having sex with other characters in order to keep him "motivated." To me at least, it would make since that her punishment for him not being able to hold up his end of the bargain would be eternal blue balling. For a porn game all about making a deal with a succubus, it makes since that the main punishment for the player failing to complete their harem before the deadline would be being forced to live with the consequences of that curse. Otherwise, the existential threat of your soul being taken back to Hell doesn't hold much weight in a world where biblical theology isn't exactly being strictly adhered to (I doubt you're really gonna explore eternal damnation in much depth).
And yeah, obviously I wouldn't be suggesting this if it wasn't one of my kinks. Again, this is just a suggestion, but the scenario of the MC being forced to spend the rest of eternity being teased and denied by Lilith does tickle my fancy. Something about being reduced to a squirming, desperate mess by a sadistic Dom just hits different, ya know? (And yes, I am speaking from experience.)

I know that there will be a few ending options, but I need to talk with my writer and eventually the coder I'm brining on to see what's doable. We're a ways off from this but I'll keep it in mind - and yeah, it's cool to post ideas here, although I'm more likely to see them on the Discord as we have a channel dedicated to player idea submissions
There was originally a 30 day time limit, I removed it after v.02 as players felt it created too much of a time crunch and, with no way to complete the goal with girls' content not being in yet, it didn't make sense to have it in.
Maybe when the game is complete I'll reintroduce a 100-day time limit or something
I think it's fine to have a time limit personally, but I know players tend to hate anything that feels like a crunch. Even the OG fallout gets flack for it's time limit, even though it's incredibly difficult to actually hit it. You can always make it toggle-able though. Most players seem fine so long as it's not forced on them.
