Does anyone know why the character is just in black is this a bug? It seems that I should be able to see the character better (in color) judging from the pixilation on the genitals.

That's a setting.
It's called something like "domination sillouette."
Does anyone know why the ghost girl won't show up for me throughout the fight? Also, the image pops in momentarily as they die.
Pretty sure it's intended, though I don't remember how to deal with it. IIRC something to do with magic.

They're bugged visually.
One of the bosses is bugged like this during a cutscene too.

nope. It's otherwise harmless, though it does make one scene confusing, so I just ignored it.
At the risk of sounding like a netnoob, do you know what the password is to the files? I'm not seeing it listed anywhere on the site or the download page
