You don't need to start a new game. Old saves should work fine. The game is open world after the first 3 initial maps.
We can change skin/hair color, hair style, and a couple other things I can't recall right now.

Because we've had like tons of content/art finished but needed more time to put everything together. So for at least for the next couple of months updates will be very regular.
We have this planned and even have some art assets for it but it might take some time since it's not a trivial thing to implement and it's not top priority right now.

Think there’s an option for npc dick sizes when starting a new game. the “small dick" option is honestly just a more realistic sized big dick and small balls while the "big dick" option is an over exaggerated one. If it’s still an issue then I can tone down the dangling balls on the "small dick" options on future characters.

I'm actually talking about a balless option where you can see that labia skin is developed into labia rather than scrotum with the slit still fully open rather than closed at the embyro stage. Balls not fully dropped, staying on the side, making a bulged labia, or still staying internal with flat labia, no difference from a true woman except for the clit is developed into penis with a glan.
