Once you beat the game enter into the Witch's house, have a conversation with her and ask her to take a shower. Head up to 2F and before you enter in the shower hover around the bra and panties sticking out and a prompt should appear to examine it and you will get it from there.

just go back to whr you fight the witch alter, she got phase 2

i may have messed up, didnt go straight back to ninja girl after snake boss, now finished game and never saw her again. atleast the crab can unlock her H-scenes.
