I have no idea what sunshim88 meant by "the lacy guard" but in front the church, on the seaside part, there is a white haired girl. Speak to her, ask her troubles, she will give you money, and then outside the left side of the church, at the end of a corridor, the medicine guy will spawn. If you don't talk to the white haired girl outside he won't spawn (my left tbh)
