I cannot for the life of me figure out how to get the CGs for prostitution - I am sure I have interacted with every npc and visited every location many times at every combination of lewdness, desire, and karma level, and yet I have found nothing. What am I missing here?

Never mind I figured it out. In order to engage Alice in prostitution, you need to have at least 30 karma, 200 desire, and NOT be horny. If you're horny, Alice will instead proposition whichever man you interact with for unpaid sex.
