clinic and library give magic xp to unlock more spells in your room in church
as for extra scenes in clinic, IDK(I only did semi pure run), try increasing lust level first

I've maxed out lust level and still nothing, starting to think it isn't really a feature to this game after the most recent updates.

if you are talking about rustium an adventurer inside explorer's association tells you that there is ruins which has a door with a same aura as the key you have after that you can see the nameless ruins on the map

not all dungeon i think , so far u get sex scene after defeated by goblin(or froslin ) in
1. froslin dweling
2.froslen large village
3. tentacle room in abandoned mine

Well I am not sure what happened, but I cannot even get to the title screen the screen just stays white and nothing loads in. But it never crashes so I am unsure what to do I will redownload it I guess. Added the translation in case it mattered but it didnt and the title stayed in japanese.

Do they daily churchgoers never show up if you play on hard mode (shorter time/more debt)?

They always show up, I think that you haven't interacted with the altar which is required for them to show up.
If that doesn't work your file is bugged.
