Considering that the Amulet of Blair didn't work back in the Demon Lord version of 0.6 until someone pointed it out and Cari's excuse was that he forgot (he later patched it in one of the tier releases prior to 0.6's Public release), it would be equally hilarious I think if the letter is supposed to be used in this particular moment but once again Cari forgot about it.
Think of it as hot-cold game. There is a place somewhere on the screen that is the spot you need to click AND HOLD the mouse button on for it to open. The closer you are to that spot, the more the lock will turn before stopping. Pick a spot at random and click+hold on it. Does the lock move? No? Then click on another. If it moves but then stops and starts shaking - stop holding the button and click somewhere close by. Did it move further? If no, go in the opposite-ish direction. If yes, keep going until you find the spot that'll open it.
Yes, the death have to pass the river to reach afterlife. in greek mythology Acheron is a boundary river separating the living world from the Underworld and there are 4 more rivers in underworld. so my theory is Temple of Acheron will connect all the rivers together which will also resolve the cycle. the reason i am claiming that the Temple of Ten Rivers is actually the Temple of Acheron because it's name and the symbol of Ouroboros cause that's also imply of cycle..

Edit: Unless this is intentional and that Orion forgot about the letter due to his low wis stat working as intended.
