I understand changing julia's name to sister would make scense but roxanne is a call girl for the mc, changing her name to mom or mother makes no sense at all since julia and roxanne have never meet untill the day at the mc's apartment?
Please explaine the activation of the incest patch please... if all it does is just changes there names it makes no scense for rosanne at least or is there actual content to be unlocked buy changing there names to 'sister and mom or mother". ?

It does not change their names, but their Title (the word in the brackets). It activates the incest route. Vicky is the escort girl. Roxanne is the not yet introduced mother.
