I don't know much about coding either, but writing wise, when introducing Keren, you can do,
"I received a call from Keren, my..." [what is Keren's relationship to you] and then let the player type out the relationship like with the MC's name, and any future mention will be what the player entered in the intro,
e.g. a couple lines later.
"Well, back to Keren, who is already late. She is my [player entered relationship]"

Honestly, i'm not a dev, just someone who's played lots of lewd games for the past couple years, so i can't say much on it.
But doing it this way will just skirt Patreon's ToS, and you can claim ignorance, in that it's what the individual players themselves chose as the relationship type.

regardless of whether you decide to do it or not, your game will probably end up in F95Zone's ongoing thread for incest games, and someone will create an unofficial incest patch.



basically, the moment you've written them in a way that they appear to be related, someone will properly turn it into incest
Many games in the list either is straight on incest properly written by the dev, gives players the ability to chose the relationship, or someone comes in and adds an incest patch. the ones written by the dev, are mostly devs who don't use Patreon, or stopped using it, or posted an official incest patch outside of Patreon.

On a side note, consider using SubsribeStar as well.
