One of the most impressive things DM has done in this game is reflect some of the realities of prostitution. Ask any prostitute if they experience orgasms with their customers and they'll laugh at you - unless you're their client at the time, in which case they'll fake one for more money.
Contrary to what we see in porn, women don't magically become turned on and experience orgasms while having sex with men for whom they feel zero attraction. Its a different story at home with their partners, but sex at work with repulsive customers is about as much fun as a friend dragging you to their gym session for support, when you'd much rather stay home and play porn games.
As noted by agn25, Monica comes across as asexual to some degree, because she's never enjoyed sex and feels there are far more important things to take up her valuable time. That gives her an advantage over other women forced into prostitution, because the only emotional trauma she has to deal with is disgust from letting her inferiors touch her, which is compensated by her need for the money and repeated vows to make them all suffer terribly, when she finally gets her revenge!
Agree that Claire could be the key to a different ending, because she's probably the first person Monica has encountered that's willing to help her without demanding something in return, especially when she's at her most vulnerable. Whether this leads to an epiphany from Monica and a possible interest in sex as a form of intimacy remains to be seen, but it'd be interesting to watch.

The answer is in the name itself. Bitch Monica behaves like a bitch to everyone and good Monica behaves in a friendly and nice manner. In bitch mode you can humiliate, fire employees, treat people like shit etc. I didn't go through the good mode, so you may want to ask people about the events takes place under good Monica.

First of all, what you have written does not help in any way to understand what has happened. Did you pass episode 2 and you can't use your save from episode 2 in episode 3 or what? Describe it in more detail. At what exact stage the error occurs, the version number of the game in which it appears, etc. And preferably put all logs with errors under a spoiler.

Many of Monica's decisions in episode 1 will have global repercussions in episode 2++. Even 1 line spoken in a rude manner can have far-reaching consequences. But whether Monica should be decent or not is up to you to decide.
One prime example is the casting of the models in episode 1. You can go around being polite to the two aspiring models and not have them strip down and humiliate themselves in front of Monica, in which case Monica won't have to go through special casting calls for Biff by humiliating herself in front of him in future episodes. There's another fascinating event in episode 2, which is also affected by how you treat the two models. My advice is don't skip this content and treat the models in the casting rough.
The other decisions from episode 1 (if I remember correctly), don't directly affect the availability/unavailability of certain content (like Biff's castings), but they do affect the characters' attitude towards Monica (talking about the people she humiliated in Ep 1). There are a lot of decisions in episode 1 that affect Monica's future. But if we're talking about Monica's bitchiness or her decency as a 'parameter', then those values can't affect what Monica did in the first episode, but they can give you some leeway in your dialogue choices. Sometimes in the game there will be dialogue options that are only available for bitchy Monica or vice versa only for decent Monica. This is where in-game cheats come in handy.
