In the game, I could imagine two paths. A sinister one where she is forced to get surgery, to become a fuckdoll. While your proposition of a branch where Monica willingly becomes a bimbo sound ridiculous to me and totally out of character for Monica, the second one where she is coerced or tricked into changing her look is possible and can even be fun as one of minor paths or endings.
Actually, if you think about it, everything is almost ready for it. That old investor from the last photoshoot, Mr. Campbell or whatever his name, can persuade or even sponsor Monica to changer her appearance. I mean he is already trying to influence Monica to become "uninhibited and confident" and to promote the new course for the magazine and so on. So, using his leverage as a major investor he can try to make a "perfect model" out of Monica.
Sure, such stuff is not for everyone. But it might be cool as an additional branch and attract new people to the game. Those who don't like it, should be able not to play it.

Actually, I think it is about time for Decent Monkey to split the game into independent branches so that there will be more possibilities to develop the story. Probably in Ep.4.
