You have to lose to each enemy 3 times, clothed defeat, damaged clothing defeat, and naked defeat.

Usually, it's in "C:\Users\{your user name}\Appdata\Local\" in a folder named "Sakura-R" or similar. "data.dat" is the name of the save file. Delete or move it elsewhere.
