Make sure you are holding the gamepad stick up/middle/down appropriately then lightly and quickly tap the buttons. You have to hold the arm or leg button so it moves whichever of those two you're trying to break loose but lightly and quickly tap the other button. Can't remember at the moment which one that is. But Innocience is correct; if you look in the journal it shows you which buttons have to be used. Point being, if you're heavy fingered or press the buttons too long, it tends to fail. I had that issue when I first tried it. Once I figured out the light taps and smooth quick movements and I didn't have any issues ever again. It almost like the game interprets too long a button push or too heavy a tap as multiple pushes on that button and it fails.
If you mean in Virginia's room, there are certain parts of the story were it is locked and you can't open it. Been a while since i played the earlier parts but I believe at a certain point it is open moving forward where you can access it.
