You can change clothes by talking to Elimina but you will only be restricted to Stormgard while wearing costumes, as for the endings, me and alot of people don't know yet on how to trigger different ending, i got the brothel epilogue but am guessing thats what everyone is getting as well.

I got Shrine Maiden Service ending. My stats was Times in heat:17 , Shrine Maiden Service:4 , Prostitution:4. I think Lewdness and Sex Defense also effecting the ending since there are 5 of them. I want to change some values via Joiplay and try to finish game again but ending is just too long for experiment.
Edit: I just changed 255 lewdness to 5 lewdness and got maid ending. I think this is true ending because she is with Emiliana.

To get school and maid costume you just need to go with Story, for chinese dress you need to do brothel Side quest and for swimsuit you need to do too much trade ( last one was in hospital). But they don't have many effects. Just one time use generally.

2) To unlock hospital you need progress in Story as well.
3) Trades starts when you enter Strombrad. I am not sure if I remember all of them. I will write the ones I remember. First guy you talked in strombrad, right bottom of Strombrad in WC, Middle of Strombrad a child, top left of Strombrad a man in bed, in fight club, middle right of Strombrad in house with blue roof ( There wasn't trade sign on top of this guy), lastly hospital.
