I hate Chattel out of spite purely because it forces them into a praying pose (which AI gen is horrible at). Otherwise, it's probably one of the best FCs, S-tier, fits all builds.
Use the SD1.5 prompts instead, unless you're using a Pony model that has no training on traditional prompts. There's a bunch of misguided shit like that in the XL/Pony prompts.
Curious about AI art (i used ol' dependable vector paperdoll), so when the slave is described as unattractive / ugly does the art also visualize them as such? what happens when there's changes on the slave's weight / cup size / pregnancy phase? or when we apply new piece of clothing?
If your model is capable of rendering ugly people, it will. Most models can't do it.

So how the AI art works is it builds the prompt in-game. This is heavily customizable in-game too on both an all-slaves and per-slave basis. Code-wise, the prompts are separated into parts (e.g. prompt for clothing, height, hair, etc. then put together).

Beauty isn't included in the prompt, but there is an outsourced attempt (which I think is dumb) to do so. It won't ever work because AI can't differentiate beauty. Some somewhat advanced models can, mostly realism models, anime models usually can't. They need to be trained to differentiate them. Either way it's not part of the prompts right now.
Edit: I use Illustrious setting for anime, and beauty isn't included in the prompt. It might be different for other settings.

When the state of a character changes, the AI art updates depending on your setting: it's either reactive (generates a new image to reflect their current state like changes in weight, hair length, devotion/fear, etc.), or static (updates every X weeks, set by you). These images are usually saved for that character, so if I have a slave that changes outfits often, the next time they change into a previously worn outfit the game won't generate a new image (if only the clothing changes).

Also, most users have "ugly" or several variations ("deformed", "malformed face", ...) in the negative prompt. It goes without saying that you should not have that in your negative prompt if you want to render ugly faces or bodies. ;)

If you just want a specific slave to look ugly or different from the other ones, then you can take advantage of the fact that the AI generation in Free Cities is very customizable: go to civitai, search for a LoRA that contains "ugly" in its description, install that LoRA in your system, then use the trigger word for that LoRA in the prompt associated with your ugly slave.

You can have a collection of LoRAs with different effects and associate their trigger words with specific slaves. That gives you a lot of options to play with, if you are willing to spend some time customizing the prompts for your slaves.

Although it already worked with it, the more recent versions of the modded version has more specific support for Illustrious as well. Illustrious models are heavily 'anime' based because they're primarily trained on Danbooru, but it's precisely because of that same reason that it is easier to work with (to me, anyway). The prompts it uses are all Danbooru tags, so if you are like me and need to customise every single slave's AI generation prompts because you're not having your slave change states that often (that you deem necessary for an image update, anyway) you can quickly generate a set of prompts you can save in a notepad or similar that you paste in initially, then modify by adding bits and pieces of pre-generated prompting and some extra custom prompting.

In terms of that, I mostly have all the positive prompts start with the usual 'best quality, detailed', etc., followed by copying over physical aspects of the slave the pre-generation came up with (e.g. 'toned, fair skin', etc.), then end it with a prompt I prepared for that run that describes the clothing I 'see' my slaves wearing that run, which I simplify by having all my slaves wear the same thing pretty much. My negative prompts start with the usual 'low_quality, deformed hands', etc. before copying over a part of the negative prompts that's pre-generated (mostly the part that makes sense).

That said, it can still take quite a while for it to generate according to what you want, but for Illustrious at least I find that if the prompts don't change much, the generated art won't change much either - maybe just the backgrounds might be inconsistent. Using Illustrious models that are heavily trained on a specific art style help as well if you're looking for art generation consistency.

Others have said it but many models aren't trained on how to create functional ugly and when you try to push them to do so it drags up a lot of the negative training data and you get nightmare fuel malformed abominations even when running a setup that does additional passes trying to clean up artifacts.

What kind of complications?
Not entirely sure, but I have a feeling that it has to do with the whole 'Womb Capacity' thing. For the Body Size aspect, that has more to do with the modded content where being younger means being smaller, which also has an effect on Womb Capacity. As for the Multiples aspect, it has more to do with Broodmother related content, especially if you're going full Repopulationist and having your slaves be pregnant with 50+ children. While they're having pregnancy complications, unless they are already heavily devoted to you and/or trust you, they'll naturally be terrified about their situation (if they're devoted/trust enough to their Master, though they know they might not make it due to their pregnancy complication, they'll accept their fate).

From experience with related content (seeing as you can get a Broodmother slave from one of those 'Holy Mother' cult events), the pregnancy complications can range from just permanently gaping vaginas (or anus? I don't touch any mpreg stuff at all as I always play with 'females are female slaves with some exceptions (futas)), to actual death of the slave. If their pregnancy, whether it involves multiples or not exceeds natural Womb Capacity for their Body Size, they will suffer Injuries every week that scale with the severity of the difference. This can end with their baby(ies) literally exploding out of their body as their lower abdomen is no longer able to hold together after being stretched to the limit. In most of these cases, especially with Broodmothers, if they're at risk of dying from it (and you actually mind that they're dying from pumping out babies like a baby making machine), you can most of the time fix it by doing a cesarean on them. You could probably let them natural birth them all, but I don't know if this can result in them getting injuries or dying from ruptering their vagina or something.

The only edge case where you might have a slave where you could have a pregnancy complication take place due to modded content is:

Content controlled by that flag includes polyhydramnios, miscarriage due to low maternal age or high womb volume, premature birth (less than 30 weeks under ordinary gestation rules), womb-bursting (if extreme content and hyperpreg are both on), and fetus-scrambling from deliberate ravishing with a truly giant dick.

With complications off, pregnancies will always proceed normally with no danger to either fetus or mother, no matter how extreme the situation.

I tried to do it and it's not working. Anyone can share a guide that really works?
Would you mind sharing which part doesn't work, e.g. any relevant error messages? The guide worked the last time I tried it.

In case you're using Automatic1111, did you follow this step in Automatic1111's installation guide?
Install (Newer version of Python does not support torch), checking "Add Python to PATH".

I'm curious: those of y'all who use the Incubator and\or Nursery, what "rules" do you have for which babies get put in them?
I really need to buckle down and progress my playthrough, currently pretty much any slave that gets pregnant their baby is kept but only my favorite slaves or ones that end up on an interesting story of falling in love with their friend slave get to be pregnant.
