Although the posts are locked behind a paywall, the preview on the Patreon says you must make a mistake 5 times in a row (no more, no less), which I assume refers to the ass expansion jutsu scene. I get a 'beep' sound when I make 5 mistakes in a row, but completing the jutsu afterward seems to give the normal animation anyway.

Yes you will see the normal animation, but then there's a second animation that happens afterwards.

I'm a bit confused on how to activate Ino's secret scene. I tried clicking her back but I only get one sound notification and don't get anymore no matter where I click on her back. What am I doing wrong?

If you get the notification sound, that means the next scene will be the secret scene. It doesn't show up immediately after you click 4 times, you have to complete the current scene.
