Try lifeplay, it's abandoned but has a lot of content and you can configure it to fit your play style plus there is the added bonus of being able to choose different cities / places to do it in.

You can if you beat them up hard enough, even important story npcs like the first ogre sister we meet are fuckable since they'd all reset when you re-enter the map unless you lock them in your dungeon.

If Ostim is still a thing for skyrim you can easily use that. The mod is super easy to install and has more content than most games of the type your searching for. And yes, you can fuck EVERYONE and EVERYTHING including monsters.

Your other option is Modded fallout 4.
