i don't understand why he separates premium (gold) and standard version, it just adds more work to do and little progress cause he has to make 2 different scenes for 2 versions lol

I mean, the reason is because he makes more money this way almost certainly.

Lots of dev do a normal (free/public) and a an extra/extended version, it's to make some extra money. If they are on Steam the extended/extra is always the one that is there. And most of the time it's either an extended sex scene, or another story event added, or different angles.
