If I am understanding you correctly, then you had it right with the second option. But either way, here is the "canon version" of the relationships in the game: The MC is related to Nana (also known as Alison), Amy, Elizabeth, Kat and Sophie. Jamila is not really the MC's mother in any way other than a symbolic one, since the MC and Omar (Jamila's actual son) grew up together and Jamila is a friend of the family, so she often helped take care of the MC during his younger years.

So there you have it, buddy! The wonderful, incestuous relationships of Grandma's House in a nutshell. I may have forgotten one or two others, but I figured I would keep it to the "most important" characters of the game.
