I really hope that I'm misunderstanding the scenarios presented, but to me they don't fit right:
  1. Two guys
    1. This should be MC plus two guys, so MMF. The variables set are either "san_mmf" for Sandra or "lily_mmf" for Lily. That's great, as it matches what her asking for a two guy threesome would entail, two other guys.
  2. Two girls
    1. Based on the above choice, this should be MC plus two girls, so FFF. But it's not, it's treated as FFM. The variables set are either "san_ffm" or "lily_ffm".
  3. Both
    1. Initially I assumed this would be MC plus one female and one male. As in both a guy and a girl. But it's a choice to request two distinct threesomes: one with two guys AND one with two girls; so two threesomes in total. Sienna asks which of the two choices (two guys or two girls) she wants to do first.
In one scenario, I chose Lily and 'Two Girls', then the conversation wraps up. After leaving Lena's house MC asks Lily about the threesome and she is quite excited. Lily then asks if the other person is a guy, to which MC says yes. But I chose 'Two Girls', so shouldn't it be MC plus two girls?

Unless specifically stated otherwise, I would always take a threesome with two guys to mean "two guys and a girl" and a threesome with two girls to mean "two girls and a guy". The "both" option there fits in with that, too. So it's "what is the makeup of the threesome you want", not "what is the makeup of the other two joining you in the threesome".

If it was how you read it, I would expect wording like "do you want it with another girl and a guy or with two guys? Or for three girls it would be "lesbian threesome" or "with three girls/two other girls."

To me, it's the context of Sienna's phrasing of the question:
Python: 
a "I've been wanting to try a threesome at some point."
si "And by that, do you mean with two girls, or two guys?"
MC says she wants to try a threesome. Sienna asks "do you mean with two girls or two guys?" The context of that question, to me, is "what two other people do you want in the threesome?"

The choices are further convoluted because the context of the MC's position in them changes:
  • For the 'Two Guys', the MC is NOT included in the context of the answer. She wants a threesome with two guys as well as her.
  • For the 'Two Girls', the MC IS included in the context of the answer. She supposedly wants a threesome with her, another girl, and a guy. This is very different context than the above choice.
  • For 'Both', it's again an entirely different context. This is the MC wanting to have two different threesomes, Option 1 AND Option 2. Sienna confirms this when she asks which of the two MC wants to do first.
If the plan is to keep her position within the threesomes different for each answer, perhaps it could be clarified with something like:

Python: 
"Threesome":
    a "I've been wanting to try a threesome at some point."

            "Two girls (FFM)":
                jump ffm

            "Two guys (MMF)":

I am disappointed there is no FFF choice, for at least a couple of my playthroughs/saves.

I can definitely see how you read it. I still would always read it to mean "should the threesome have two guys, or two girls", as opposed to "should the other two in the threesome be..." There's no science as to why I read it that way and not your way, so I can't explain why. It just feels right, I guess?

I like the graphics in this game. Very life like and detailed. Unfortunately the game is too vanilla and "we are all good" for me. I would have liked for the first guy to come as soon as his dick got halfway into her pussy. Some BDSM and anal scenes would also be nice. And what about her not being on birth control, and a guy giving her a big unwanted creampie? Should give some nice angry scenes. I wouldn't mind her doing prostitution on the sideline and maybe being raped by a client or fan too. Without any of this, it's just another average feel good AVN, although with above average graphics.
