I had completed HH 18.x as far as possible. I deliberately avoided the entire Beta 19.x series.

My question is:
With all the changes, including to previous events, should I start a completely new run or keep my previous progress and “only” experience the V19 events?
Any recommendations?

Thanks in advance.

I too waited for the public/final release, and a new run is probably a better call.

If you continue from a the 0.18.x save you'll immediately hit event triggers. It felt inorganic compared to earlier content, but I wanted the story progression.

I'll probably do a re-run at some point. We've got a fair amount of time before we're likely to hear about a new update, and even if you waited to start a new run until just after we hear about the first beta release, you could probably get it done in time for the public release.

The slum events were interesting, looking forward to where all that goes. I also realise that this event was fairly heavy on the scenes, so as someone who really doesn't care about them (wanted the story progression, skipped every lewd scene), it felt fairly short. I know most/many want the scenes, so I don't hold it against Runey, just an observation for anyone expecting a huge amount of reading/story.

The closest to NTR is an event where other characters have sex with what are at the time their elves, most of which you later acquire and can have sex with.

most of the story suffer
(writing as of 0.18.2; I've just downloaded 0.19 but haven't yet been through it)

I'm not sure about "most of the story," but the story-telling is definitely hampered by the sandbox format. The most egregious example has to be the major events with Lin and Felicity during 0.18--both events end with the character in question away from the hotel. Then click, you're back at the hotel, and there she is; you and the other characters can interact with her as though nothing had happened (and that ignores the fact that the entire premise for Felicity's event is false--the "dirt" they'd found just isn't there). But then at the next events for these characters, they're presumably going to be back wherever you left them.

There are countless other smaller examples--either things like you mentioned, where characters should know (and thus act on) things they don't seem to know; or where dialogue refers to things that haven't happened yet (I had a date with Maria where she referred to my literally owning her--but that story event hadn't happened yet). It's possible to code around things like this, but apparently it hasn't been done carefully enough.

Since there will likely be more tweaks done to earlier scenes in the game in the future, it would be better to wait until the final release of the game comes out to do a full game replay.

maybe most were wrong word. english isn't first language.
"a lot of the story."
would be better. some stuff is just wierd if you play the game in one go and you have chars that never talk with each other or act different because the time line isn't like the story want them to be. Or even chars that feel out of place.

other problem how we not talk with others. Vanessa could be a good help with Lin story. Or Rin with the Twins. Or Rin with Asley.

and that i fear some get to big. I mean we have how many storys that could place ourer MC in a dark hole?
We have the Android.
The Sanctury.
The High Elves.
The Owl clan.
Hana.

and some of them feel like they don't know/interact with each other.

Damn, this game is really awesome. I thought it would be like Mythic Manor, where you run a hotel with girls. But this game has truly been something else. I really like the story and the characters. They're well-written, with each girl adding depth to the story instead of just being a sex object.

I played this game for the first time on version 0.19. In the main story, when the MC is kidnapped by Jia, the girls from the hotel try to rescue us. I remember seeing Felicity in the background during that scene. But how could she be there if she was taken by her dad? I think the dev made a mistake here. So looks like version 0.20 will have the story progressing with the high elves queen coming into picture.

Also how long does the dev take to release a update??

Just be at level 9 and go to the sanctuary. Unless this event is bugged too, I've seen some people unable to trigger it on discord.
I have the Pre release version and it has the bug where Nia's event is not triggered. I am hoping that this is fixed in the Full version

Edit: I downloaded the full version and I am still not able to trigger Nia's event. What is this event?

Mine's lvl 9 but I still can't get it to work at all. Tried using a mod to redo her quests from the start, but nothing.

So I think that Android's lvl 15 is borked in 0.19 since it's not proccing at all even if I stand in my room and skip the time to night.

Yes I will! I'd be on it already but my workstation is currently dying a slow death, it'll have to wait until the weekend.

These are explicitly a compromise solution to the problem of episodic releases - once the game progresses far enough, they'll both return to the hotel in the natural course of events, and you'll be able to continue grinding their non-story scenes, and progress other characters' storylines through scenes that involve them. With the updates only progressing the individual stories partway through those characters' absences, if there was no way to interact with them until they returned, you'd soft-lock a bunch of stuff until v0.22 or v0.37 or whenever the absences ended.

Having the characters pretend to be around while they're absent, until an update brings them back, means players can't soft-lock themselves, without forcing it through making them complete absolutely everything else before being able to play the disappearance events (and even that has potential issues if the disappearance lasts several updates).

So, yeah, Felicity and Lin being kinda around for the purpose of avoiding soft-locking other storylines until some future update is something I'm very okay with.
