i think i remember Runey mentined Mc Sister and that at some point she would arrive in the Hotel,but i dont remember when/where exactly it was said as well as when its suposed to happen in the game

To the best of my knowledge Runey didn't mention it, it was a dev that collaborates with him that mentioned it back in 2019, but since it hasn't happened by now I doubt that it will at this point (especially given the new issues with this kind of content since 2019).

If such a sister were added, due to patreons rules the MC would be unable to do anything with her, unless someone created a patch. But there are far better options for getting that last room, so the sister would be unlikely.
A lot of people lean towards Hazel getting it, or perhaps Kali's stepmother.

I personally think it is best used as TheDevian's private head patting room.
Autumn is already used to the squeaks, squeals and giggles that come from that room every night.

MC's sister is in theory the only family he has left behind so of course it must be a character that has to make an appearance at some point. It's important to the lore and Runey takes care of such details. Neglecting her would be a big time waste. Maybe just a scene about a visit at the hotel would suffice. Imagine the reactions of each of the heroines to her so long-awaited sister-in-law. Man, the day that happens will be the day I become INSANE on Patreon

They are clones of one another if i recall. Perhaps new players could get our perception of the "truestory" that they be for example twins instead. Just in such a way the MC's childhood friend can come and move into the final room and be you know related in perceived "truestory".

It should happen automatically unless you move or delete the persistent save folder for some reason.

You do know, that is irrelevant what the law will define, it is the ToS of Patreon that will define (or let open) what is incest and what not.
Of course, your free to discuses the details with their greatest Trust & Safety team that have also defined that Ashley in a college uniform doesn't look enough like an adult and Runey needed to change that.
Therefore, I'm really optimistic for this new question.

did you play the beta release for this update? if so, it causes problem for previous saves which might be what happened in this case

If you still have v0.18, try saving in your room before moving to v0.19. Saves made in other places or during events can cause errors when changing versions.
